Job Title: Lead Assessment & Review Officer

Salary: £12.19 per hour

Hours: 37 per week, 52 weeks

QRS are currently recruiting on behalf of a college in Lincolnshire for a Lead Assessment and Review coordinator on a temporary basis until the end of August, with a view to be extended into the new academic year. 

Overall Purpose

To lead on the co-ordination of the annual review of Education, Health & Care Plans (EHCPs) for students across the College and ensure progress towards educational outcomes is being reported on at least termly. In addition, the post holder will lead on any new applications for students who require an assessment for EHCP. You will be required to consult with Local Authorities and host annual reviews and any other tasks associated with the review process.

Accountabilities

  • Lead on the development of the assessment and review processes linked to ECHPs. Co-ordinate the caseload between the Assessment & Review team.
  • Liaise closely with curriculum staff, Lead Learning Supports, Transition Officer, students and their parents/carers/ward and collate information which will contribute to the individual Annual Review of their EHCP.
  • Develop and implement cross College processes that demonstrates an ‘Assess, Plan, Do, Review’ approach and lead on writing high quality summative reports from the information collated.
  • Develop and maintain positive relationships with Local Authorities, ensuring that requests for information are met to the identified timescales as detailed in the SEND Code of Practice.
  • Create and co-ordinate an annual review schedule for all EHCP students across College, both on mainstream courses and in the Skills for Work & Living (SfWL) area, taking a lead as chair whenever possible.
  • Undertake Assessment of Needs interviews as identified.
  • Liaise with SEND and curriculum colleagues to complete formal consults to Educational Health and Care Plans.
  • Liaise with Lead Learning Supports, Transition Officer, Programme Co-ordinator – SfWL and SEND Co-ordinator; to source an alternative chair for EHCP and student centred meetings as required.
  • Ensure all relevant individuals are invited to annual reviews, including professionals from Social Care, Health Care, Education as well as parents/carers/ward, and the young person. ∙ Undertake student assessments/observations in school settings and during the College programme, as required.
  • Work with Finance Officer and Support Co-ordinator to cost support provision for individual support packages and for high cost students.
  • Participate in joint working groups in order to share good practice and develop networking across the county. Disseminate best practice within the ALS team.
  • Update the ALS Team with appropriate training on EHCPs as appropriate. ∙ Support new Lead Learning Supports in the understanding of EHCPs.
  • Attend ALS and curriculum area meetings as requested.
